School Survey Forms
The content of the survey questionnaires for primary schools and high schools was identical. However, there were variations in the forms for each of these settings to allow for differences in access to the students and in recognitions of the larger numbers of teachers that typically have access to students enrolled in Year 8 and above. There were three separate questionnaires used in the school survey – one at the school level, and two student-level questionnaires.
These forms were designed to gather information on the demographic characteristics of participating primary/high schools, the composition of enrolment, the composition of teaching and supporting staff, and what professional and curriculum development there is in activities pertaining to Aboriginal students. The final portion of these questionnaires sought to identify aspects of the schools which facilitate student’s educational, vocational and social competency.
These forms, to be completed by the primary/high school principals, were intended to report on the main language used by the student, overall academic achievements made by the student in literacy and numeracy relative to all students of the same age, information on the student’s enrolment details, including school attendance, use of boarding facilities, and the use of support services by children with special needs.
Measuring behaviour, non-verbal reasoning and English word definitions
This questionnaire was used to identify and measure risk of clinically significant emotional or behavioural difficulties. It also contains a brief test of English language proficiency and instructions for completing a test of non-verbal visuo-spatial reasoning.

This booklet was used by all primary/high school students to record their answers to the non-verbal reasoning test. For high school students this test was administered through the School Counselor/Psychologist or Year Coordinator Year Head/Form Teacher or Tutor’s Questionnaire; for primary school students it was administered through the Classroom Teacher’s Questionnaire.
