Child Health Questionnaire (0–3 year-olds)

The purpose of this form was to learn about the health and wellbeing of Aboriginal children under the age of 4 years. It asks questions that will reveal how children are growing, learning and behaving. Also focusing on health problems experienced by children and the services they need. This information was needed to understand what is important for Aboriginal families and what needs to be done for their children to get a good start in life.

Child Health Questionnaire (4–18 year-olds)

The aim of this form was to learn about the health and wellbeing of 4–17 year-old Aboriginal children. It asks questions that will reveal how children are doing at school, their social activities and behavior. It also examines the health problems experienced by children and the services they need. This information was needed to have a better understanding of what is important for Aboriginal families and how they help their children to get a good start in life.

Youth Questionnaire (12-17 year-olds)

The questions on this form were designed to help the survey team to understand how things are for young people. The questions focus on health risk behaviours, diet and nutrition, emotional and behavioural difficulties, suicide, and other experiences and exposures. Information collected on these topics is designed to be used to improve health, education and other services for Aboriginal youth.

Carer’s Questionnaire (Caregiver 1 Questions)

This form is for a parent/caregiver of Aboriginal children aged 0–17 years. The health of a parent/caregiver, housing and family life is important for children’s health. Questions on these topics, along with Aboriginal culture, language, schooling and employment, are the focus of this questionnaire.

Carer’s Questionnaire (Caregiver 2 Questions)

This form collects information about the second parent/caregiver of Aboriginal children aged 0–17 years. The questions on this form are identical to those on the Caregiver 1 questionnaire.

Household Record Form

Collects information on the number of people in each survey household and how they are related.
